Influenza vaccinations will be available for anyone 6 months of age and older!
Location: Blythe Academy Cafeteria
(Medicaid and Medicare Plan B are accepted.)
Vaccinations will be administered by a qualified health professional.
Injectable – licensed for anyone aged 6 months and older
Intra-Nasal – licensed for non-pregnant individuals who are healthy and aged 2 years through 49 years.
** INSTRUCTIONS **
You can pre-register for your flu shot two different ways.
1) Call toll free 1-866-358-2878 extension 228 between 9:00am and 4:00pm Monday through Friday Eastern Standard Time.
2) Online at https://secure.flubusters.com/home.aspx and follow these directions.

Click on Clinic Search.
Select Blythe Academy location.
Select desired time.
Select Flu Mist or Flu Shot.
If more than one person is getting a vaccination, add another time or click on View Cart/Check Out.
Enter your information and Submit Payment.
Deadline to pre-register is Thursday, Oct. 8.
Walk-ups will be accepted for the injectable vaccination on an availability basis.
